JJ Keller created a document for the intrastate definitions for all 50 states: here is what they wrote about indiana -

    Indiana

    Adopted Parts 40, 375, 380, 382-387, 390-393, and 395-
    398
    (with a few modifications)

For-hire carriers operating motor vehicles exclusively in
intrastate commerce that have a gross vehicle weight, gross
vehicle weight rating, gross combination weight, or gross
combination weight rating of 10,001 pounds or more.

Private carriers operating motor vehicles exclusively in intrastate
commerce that have a gross vehicle weight, gross vehicle weight
rating, gross combination weight, or gross combination weight
rating of 26,001 pounds or more.

The adopted regulations apply to any size vehicle (for-hire or
private) transporting a placardable amount of hazardous
    materials.

    10,001
    and
    26,001

So if you're private motor carrier and under the 26,001lbs threshold then no medical card is how i read that.
